Sinner, "microtraumas": the injury that holds him back, the expert speaks | Libero Quotidiano.it

“I’m not ready to compete yet.” Jannik Sinner withdrew from the Cincinnati Masters 1000 after undergoing further check-ups in Milan, just days before the scheduled departure for the United States. The problem remains the right knee, which continues to cause discomfort after the break following the Wimbledon triumph. A cautious choice, with the US Open now in sight, but it has reignited questions about the physical condition of the world number one.

Explaining what might be behind a problem of this kind is Andrea Bernetti, full professor of Physical Medicine and Rehabilitation at the University of Salento and general secretary of Simfer, interviewed by Adnkronos Salute: “When a tennis player complains of a knee problem without having suffered an acute trauma, as seems to be the case with Sinner — explains the expert — we are probably facing an overload pathology.”

The reason lies in the characteristics of modern tennis: “Continuous stresses, jumps, stops, slides, and violent changes of direction.” Repeated microtraumas that, according to Bernetti, can exceed the body’s ability to recover. The numbers help to understand the phenomenon: injuries to the lower limbs account for between 31% and 67% of all those recorded in tennis, while the knee is involved in 10-15% of cases. And among these, 60-70% do not result from sudden trauma but from overuse problems. Possible conditions also include meniscal injuries: “They represent about 10-20% of all knee injuries in young athletes,” Bernetti points out, recalling how twists and changes of direction subject the menisci to strong compressions and stresses.

The expert clarifies, however, that it is not possible to automatically link Sinner’s case to a precise diagnosis. Possible therapies depend on the nature of the problem and may include “therapeutic exercise in water,” instrumental physical therapies, injections with hyaluronic acid, PRP, collagen, and ozone therapy. All, Bernetti specifies, “must be evaluated in a personalized manner.” The message, therefore, is mainly one: without knowing Sinner’s diagnosis, it is not possible to establish the real extent of the problem. The withdrawal from Cincinnati, however, represents a cautious choice, in view of the commitment at the US Open.
